Welcome to New Providence Middle School! The following is required to complete registration:

Birth Certificate

TN Shot record- 30 days to get this

 Parent’s Government Issued Photo ID

Custody paper work (parenting plan, POA, etc) We must have this before the student will be enrolled.

Two of the following items verifying proof of residence in the New Providence zone:

1. Mortgage Verification (with name and address)
2. Tax Receipt (property taxes or personal property taxes)
3. Current Rental Agreement (naming tenants)
4. State/Federal Records (Federal Tax, Soc. Sec, Child Assistance Records)
5. Utility Bills (electric, gas, water/sewer, landline phone, waste disposal)
6. Current Car Insurance or Vehicle Registration (with name and address)
7. Internet, Cable, Satellite (with name and address)
8. Employment Records (with name and address)
9. Government Issued Photo ID (with name and address)
